Arizona Dachshund Rescue & Sanctuary Adoption Policy

It is the policy of Arizona Dachshund Rescue and Sanctuary (AZDRS) that all
potential dachshund adopters complete the Dog Adoption Consultation Form and
complete an interview with an Adoption Coordinator in order to be considered as
a forever home for our dogs and puppies.

AZDRS will determine the best home for each dog or puppy based on the
application and interview process. After review of the adoption consultation form
and the completion of the interview with the Adoption Coordinator, AZDRS may
recommend a different dog or puppy that is more suitable to your household.

Completion of the application and interview process does not guarantee an
adoption approval or guarantee the adoption of a particular dog or puppy.
AZDRS is a privately run, non-profit organization and reserves the right to refuse
any adoption that is not in the best interest of the dog or puppy, based on our
principles, policies, and standards.

Please read the following instructions carefully.

1. Please fill out all pages of the AZDRS Dog Adoption Consultation form as
completely as possible. The submission of a complete form will expedite
the review process.

2. After the form has been completed, please print a copy of the completed
form for your files. After you have reviewed the Dog Adoption Consultation
Form, rename the file and save it to your hard drive.

3. To submit your completed Dog Adoption Consultation Form for review, click

of the completed form from your hard drive. The AZDRS Adoption
Coordinator will contact you as soon as possible.

At Arizona Dachshund Rescue & Sanctuary, we believe there is a home for every
dog and puppy we foster. Our foster program provides us the opportunity to
evaluate the behavior of our dogs and puppies in a family environment, allows us
to address any unwanted behavioral issues, and allows us to provide adoptive
parents with all the information necessary to make an informed adoption decision
and insure that their new dog or puppy will be a good match with their family.
